I'm seeing intermittent, inexplicable failures that may not even be related to slapd. I'm going to try again today with BDB 4.8.30, though.

Did you try BDB 4.8.30 and still see it? We've finally tracked down that this appears to be coming from the BDB library with 4.7.25 + patches, at least.
