As noted in http://www.openldap.org/faq/data/cache/59.html, when slapd crashes, one should provide a backtrace from gdb.
I.e.:
Start slapd
gdb /path/to/slapd pid
(gdb) cont
Then execute your ldappasswd command in another window. When slapd crashes, it'll drop back into gdb.
(gdb) thr apply all bt full
Provide that output back to the ITS.
