The add was on a user entry:

dn: uid=cs5555,ou=testPrimary,ou=mailhosts,dc=test,dc=com

not in cn=config.

I'm not sure what was done to modify the schema entry.

Ah, you're right. But slapd should not have crashed during that operation.

We still like to assert() on nonsensical results, as occurred in this guy's log.

@Chris: Could you reproduce all that with a recent OpenLDAP release?
