The add was on a user entry:
dn: uid=cs5555,ou=testPrimary,ou=mailhosts,dc=test,dc=com
not in cn=config.
I'm not sure what was done to modify the schema entry.
Ah, you're right. But slapd should not have crashed during that operation.
